How to set up your survey questionnaire

Build your survey from scratch or use EngageRocket's question library, a step-by-step guide.

Once your project is created, the next step is building the survey itself. EngageRocket gives you a flexible questionnaire builder with multiple question types, conditional logic, dimension tagging, and translation support. This article walks you through the full setup process.

Starting your survey

  1. Inside your project, click '+Create New Survey'.
  2. Enter your survey name and set the survey timeline,the start and end dates for data collection.
  3. Choose your starting point: Use EngageRocket's question library (recommended for standard engagement surveys) for a ready-made set of validated questions, or click 'I have my own questions' to build from scratch. You can access the library at any time later by clicking 'Templates and library'.

Question types available

EngageRocket supports six question types. Here is a quick guide to when to use each one:

Rating Scale

Asks respondents to rate something on a defined scale (e.g. 1–5). You can customise the scale labels,for example, from 'Strongly Disagree' to 'Strongly Agree'. Rating scale questions can contribute to an Overall Score on your dashboard. Note: As of August 2025, the same rating scale options apply to all rating scale questions in a survey,changing the options for one question will affect all others.

eNPS (Employee Net Promoter Score)

A standardised 0–10 scale measuring how likely employees are to recommend your organisation as a place to work. Ideal for tracking overall sentiment over time. Best paired with an open-ended follow-up question.

Yes / No

A simple binary question for clear, factual data where no nuance is needed.

Single Choice

Respondents select one answer from a list of options. Best used when the possible answers are mutually exclusive. Consider including an 'Other (please specify)' or 'Not applicable' option to avoid forcing respondents into a choice that doesn't fit.

Multiple Choice

Respondents can select more than one answer. You can set a minimum and maximum number of options the respondent must select. Includes an optional 'Other' field for open-ended responses.

Open Ended

Free-text responses,ideal for collecting qualitative feedback and understanding the context behind scores. Use these sparingly to avoid survey fatigue, and place them after a related rating question.

Setting up your questions

  1. Add a question by selecting the question type from the question builder.
  2. Enter the question text and customise labels or options where applicable.
  3. Set mandatory or optional: decide whether respondents must answer this question to complete the survey.
  4. Assign a Dimension: Dimensions are category tags that group related questions together on your dashboard for automatic analysis. For example, tagging questions about work-life balance under a 'Wellbeing' dimension.
  5. For rating scale questions, toggle 'Contribute to Overall Score' on or off. Only rating scale questions contribute to the Overall Score on your dashboard.
  6. Set up Conditional Logic if needed, show or hide questions based on a respondent's previous answers or their employee segment. This personalises the survey flow and keeps it relevant.

Finalising your questionnaire

  1. Customise your Welcome Message, Thank You Message, and Survey Header (upload your company logo) from the right-hand panel.
  2. If your survey supports multiple languages, go to the Translations section. Library questions are auto-translated. Custom questions, the survey title, and the thank you message will need to be translated manually.
  3. Click 'Preview Survey' to review the full survey exactly as your respondents will see it. Check for typos, question order, and that any conditional logic is working correctly.
  4. Click 'Save and Continue' to move to the Recipients page.

💡 Good to know: Your survey is saved in draft status until you complete all setup steps and launch it. Respondents will not receive anything until you explicitly launch the survey.

Something not working?
  • My custom questions are not showing translated versions: Only library questions are auto-translated. You must manually enter translations for all custom questions, the survey title, and the thank you message in the Translations tab.
  • The Overall Score is not appearing on my dashboard: Overall Score only applies to Rating Scale questions. Make sure at least one rating scale question has 'Contribute to Overall Score' toggled on.
  • Conditional logic is not behaving as expected: Double check that the condition references the correct preceding question and that the answer value matches exactly. Preview the survey to test the logic before launching.
